Existing plug-in forms do not close when a plug-in is disabled

Description

To reproduce:

  • Load the Network Analysis form
  • Go to Tools > Options > Plug-ins
  • Disable Network Analysis plug-in

The Network Analysis form should have closed

Work-around. Enabling or disabling plug-ins will require an app restart
