Define taxonomy created multiple taxa

Description

I defined a number of taxa in the define taxonomy box and pressed OK. The box did not close and I pressed OK again. The box again did not close so I pressed Cancel.

In the taxonomy grid all my new taxa were listed twice

Error is related to assigning taxa to multi-stanza configurations
